With this new software solution, BKK employees can win over patients for the DMP program and subsequently provide them with optimum care. This ensures the efficient and customer-oriented processing of the programs for patients suffering from chronic diseases.

current status of the performed DMP measures at a glance together with the entire communication history for all registered patients. Automatic test processes will inform the service employees about administrative or medical peculiarities in due time. For example, if the documentation of an intended doctor's visit is missing or a patient did not attend one of the necessary trainings, this is pointed out automatically by the ICW CareManager to the employees, who can then check this with the patient or the physician in charge (reminding). With such a rule-based evaluation of the DMP data, it is possible to provide optimum service to a larger number of insured patients with fewer employees. Patient compliance increases, the risk of unnecessary secondary diseases or complications is reduced, and less patients have to be excluded from the DMP.
